Job Summary

The Sr. Community Health Worker will maintain a caseload of 15-20 members, supporting members from assessment through graduation based on CalAIM Enhanced Care Management requirements. The Sr. Community Health Worker will also support in the day-to-day support of staff by ensuring staff is compliant in meeting required metrics and program standard. This role will provide initial and ongoing training to Community Health Workers. The Sr. Community Health Worker will also support in the growth and the development of the program by engaging with potential members and internal and external stakeholders.

Education and Work Experience

• High School Education/GED or equivalent: Required
• Bachelor's Degree or equivalent combination of education/related experience: Preferred
• Three years' technical experience: Preferred
• One year of leadership experience: Preferred
• Successful completion of a Community Health Worker formal training program from an accredited college, or other educational institution: Preferred

Essential Functions

• Maintains a case load of 15 to 20 members. Will review and prioritize referrals, and perform outreach via phone calls, home visits, and/or community-based engagement programs and connect individual with appropriate community resources, services and care.
• Serve as initial contact with escalation to Director when necessary. Will provide ongoing monitoring and evaluation of local Community Health Work team ensuring daily, weekly and monthly metrics are met ensuring engagement and timely documentation of case activity. In addition the Sr. Community Health Worker will support the completion of CPS/APS reports, grievances and health plan audit requests.
• Provides new hire training and on boarding including introduction to the program and day to day essential functions. Continues to provide ongoing training, support and education as needed.
• Will conduct local outreach to internal and external community stakeholders raising awareness about CalAIM Enhanced Care Management, marketing the program, initiating referrals into the program, and drive volume to the program from these efforts.
• Will support locally with transitions of care from hospital, specifically supporting with more complex patient cases and engaging directly with internal case management team. Will engage with members at bedside to explain and offer programmatic services and support in the transition to a Community Health Worker for length of enrollment.
• Performs other job-related duties as assigned.
