About The Role
BASIC PURPOSE OF THE JOB
Provides comprehensive, patient and family focused care coordination for medically complex pediatric patients across the continuum of care as a member of the Hematology/Oncology team. Collaborates with all team members to promote patient and family self management and to optimize the health and well being of the patient and family.

JOB REQUIREMENTS
Supervisory Responsibilities: No
Minimum Education: Bachelors Degree required; Masters preferred
Degree: Nursing
Minimum Work Experience: 2 years pediatric nursing in hematology oncology preferred.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
- Sets priorities and makes decisions of a complex nature.
- Interfaces directly with physicians, patients, family members, and other health team members.
- Maintains confidentiality of patient’s medical records.
DUTIES AND RESPONSIBILITIES
- Establishes a supportive and effective relationship with the patient/family in the inpatient setting and educates them on the navigator role.
- Completes comprehensive and on-going assessments of patient/family goals of care. Completes and maintains shared documentation.
- Identifies barriers to follow-up care and assists patient/family to follow medical plan of care including follow-up appointments, medication compliance. Maintains contact with family following discharge, reviewing medication schedule/dosing and appointments.
- Provides anticipatory guidance, clarification and education surrounding specific disease process, therapies, treatments and interventions of patient/family and team members. Facilitates patient and families understanding of the treatment plan.
- Collaborates with team members to provide emotional and education support for patient and family includes providing family with information necessary for informed decision making.
- Identifies and consults additional interdisciplinary supportive services as needed.
- Analyzes process within the health care system affecting the medically complex patient population and direct care staff and facilitates changes to maximize outcomes through quality improvement initiatives and/or research development.
- Collaborates with leaders in program develop, supports change and adapts to meet program goals.
- Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
- Participates in interdisciplinary rounds and care conferences as required.
- Seeks new knowledge and participates in learning opportunities to support the care of children with medically complex needs
- Willingly accepts any other assignment that may be requested.
- Utilizes hospital resources and time respectfully and accountably.
PHYSICAL REQUIREMENTS
- Note: Reasonable accommodations may be made for individuals with disabilities to perform the essential functions of this position.
- Light lifting, pushing and pulling is required for 1-20 lbs occasionally and frequent moving of objects of less than 10 lbs is required. Frequent sitting with some walking, standing, squatting, bending, and reaching is required. Keyboard/computer use and/or repetitive motions may be required.
