About The Role
We are hiring a Clinical Social Worker for a leading healthcare facility in Abu Dhabi. The Clinical Social Worker will be responsible for delivering high-quality psychosocial care to patients and families, supporting emotional well-being, facilitating access to community resources, and collaborating with multidisciplinary teams to ensure safe and effective patient care.
The ideal candidate will have strong experience in hospital-based social work, case management, crisis intervention, and discharge planning, with the ability to support patients across diverse medical, mental health, and social care needs.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families to identify emotional, behavioral, environmental, financial, and social factors affecting treatment and recovery.
- Provide individual, family, and group counseling to support coping, adjustment, grief, trauma, illness management, and behavioral health needs.
- Deliver crisis intervention for patients experiencing acute psychosocial distress, domestic violence, abuse, neglect, self-harm risk, bereavement, or other urgent social concerns.
- Develop and implement care plans in collaboration with physicians, nurses, psychologists, case managers, and allied health professionals.
- Lead and support discharge planning to ensure safe transitions of care, including coordination with rehabilitation centers, long-term care, home healthcare, community agencies, and social support services.
- Advocate for patient rights, informed decision-making, safeguarding, and access to appropriate services and benefits.
- Identify and report safeguarding concerns , including child protection, elder abuse, neglect, domestic violence, and vulnerable adult cases, in line with UAE laws and hospital policies.
- Support patients and families in navigating chronic illness, disability, palliative care, oncology care, mental health conditions, and end-of-life issues .
- Coordinate referrals to internal and external community resources , social services, mental health support programs, financial aid resources, and rehabilitation services.
- Maintain accurate and timely clinical documentation , case notes, assessments, interventions, and follow-up plans in the hospital information system / EMR.
- Participate in multidisciplinary team meetings , case conferences, family meetings, and ethical consultations.
- Educate patients, caregivers, and staff on psychosocial issues, coping strategies, and available support services.
- Ensure compliance with hospital standards, UAE healthcare regulations, patient confidentiality, and professional ethics .
- Contribute to quality improvement initiatives, policy development, and departmental audits related to psychosocial care services.
Requirements
- Bachelor’s degree in social work
- Master’s degree in social work
- Minimum 2–5 years of experience in clinical / hospital social work
- Experience in psychosocial assessment, counseling, crisis intervention, case management, and discharge planning
- DOH / DHA / MOH eligibility or active license preferred
- Strong communication, documentation, and teamwork skills
- Arabic and UAE experience are an advantage
