About The Role

The Nurse Coordinator for the Heart Failure Disease Management portion of the Advanced Heart Failure Comprehensive Care Center coordinates care for our patients being evaluated for general heart failure management, as well as those who may be moved into advanced heart failure therapies workups. This role will coordinate between inpatient and outpatient departments, and manage patient panels including recommending diagnostic testing, lab testing, new and investigational heart failure therapies. This role regularly educates patients on heart failure disease, treatment, therapies, medication indications and the importance of adherence, and more. This role will interface between multiple UCSF departments, review incoming referrals, and coordinate outpatient care for patients with heart failure.This position will rotate through the Diuretic Clinic as needed with responsibilities including initiating IV access, blood draws, administration of IV medications, review of laboratory results, and follow-up patient assessment.

Onsite (ambulatory and in-hospital prn) and video support for MD clinics: including coordination of care, creating end of clinic patient instructions, patient education, among other patient care duties as needed.

Clinical In-basket management

Close monitoring and assistance in titration of therapies for patients taking guideline directed medical therapy (GDMT)

Ancillary and diagnostic testing ordering and coordination

Interpret, review results of laboratory and other diagnostics and triage results to appropriate provider, including recognition of critical abnormalities requiring urgent intervention.

Education and management of patients with CARDIOMEMS-an internal pulmonary artery sensor used in the treatment and management of heart failure

  • Assist new staff with orientation and training when needed

REQUIRED QUALIFICATIONS

  • 3 years’ ambulatory or acute care RN experience in Cardiovascular nursing including heart failure disease management.
  • Strong MS Office computer skills (word, excel, power point), databases
  • Excellent organizational skills, proficiency in meeting deadlines, ability to work independently and collaboratively with colleagues, prioritize workload and manage numerous concurrent tasks in high stress environments
  • Demonstrated excellent written and verbal communication skills
  • The flexibility to orient and work at all UCSF Medical Center locations, (ambulatory and acute care-for patients requiring education and care coordination while hospitalized)

PREFERRED QUALIFICATION

  • Bachelor’s degree in nursing
  • Master’s degree in nursing
  • Working knowledge in assessing advanced heart failure, heart transplant
  • Experience with managing and adjusting heart failure medications

REQUIRED LICENSES/CERTIFICATIONS

  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association CPR certification
